A person who has been diagnosed with one of the following conditions may apply:
-Common variable immune deficiency (CVID)
-Selective IgA deficiency
-Familial Agammaglobulinemia

A person who has been diagnosed with one of the following conditions may apply:
-Hemophilia A/B (and symptomatic carriers)
-Von Willebrand Disease (vWD)
